New Year…New Goals

The celebration of a new year is an opportunity to reflect on the great things we have accomplished and to set new goals for Vets Count/Easterseals Maine in 2019 and beyond. Each year as we develop our annual report, we are inspired by the what we are able to accomplish as an agency with your support!

This year we focused on reducing costs and investing in some of our core programs to help put our organization on a sustainable path. At the core of our programs is our care coordination model that takes a practical, holistic, and person-centered approach designed to maximize individual engagement in community support systems and prevent costly crisis interventions. The model utilizes an empirically supported methodology rooted in positively impacting the social determinants of health. The result is improved individual functioning, and reduced health care costs for each participant.

In order to expand this model, we have invested in our Community Care Coordination pilot program. The goal of the program is to partner with insurers (or other payors) to provide intensive community-based outreach care coordination services to members identified as high cost/high utilizers who have not been successful with traditional case management over the phone. The goal of this collaboration is to work with individuals to target the social determinants of health to promote long-term self-sufficiency and reduce overall cost to deliver high quality care.

Our Philanthropic Arm to our Military and Veterans Services program, Veterans Count continues to be our largest and most visible programs in the state serving Veterans, Service Members, and their families when no other resources are available to them. We were one of two agencies in Maine selected to participate in the Community Integration Services program, which has greatly expanded our ability to serve Veterans across the state. This grant has contributed greatly to the long-term sustainability of M&VS and allowed the philanthropic funds raised through Veterans Count to go almost exclusively towards giving Veterans in our community a hand-up towards independence.

This year is a historic year for Easterseals as we celebrate 100 years of service to people with disabilities or special needs. We remain dedicated to providing exceptional services to ensure that all people with disabilities have equal opportunities to live, learn, work, and play at home here in Maine. Our community counts on us to be there and we are fortunate to have a cadre of committed partners, including businesses and individuals like you who support the life-changing work we do.
